1. A list of ALL your bills - the list can be hand written and need only to include the name of the creditor, the approximate balance, and the approximate monthly payment.
2. Credit card statements so we can double check creditor addresses.  Bring ALL credit card and debt statements, not just the ones you wish to include in your PLAN.  We need to know about your mortgage, your car payment(s), EVERYTHING!
3. Your two (2) most recent tax returns.
4. Your employment pay stubs for the last seven (7) months for BOTH you and your spouse (even if only you are planning to file for the PLAN, and information on BOTH your income from ALL sources - wages, unemployment, social security, disability, worker's compensation, food stamps, child support, rental income - EVERYTHING!
5. Your State Drivers License or State issued Identification for BOTH you and your Spouse.
6. Your Social Security Card or other proof of Social Security Number for BOTH you and your Spouse.
7. If you are married and you want to file alone, all the information listed above is also required of your spouse.  If you are married but separated, do the best you can to gather the information above.  Make certain to tell your attorney information concerning ALL of your property such as land, recreational vehicles, time-shares, etc.
